Patricia and I discussed the inconvenience of telemedicine and how it also provides a gateway to security and privacy issues such as patient privacy confidentiality, and HIPAA violations. Patricia suggested that all protocols that are following during in-person appointments should be followed during telemedicine appointments as well. Doors should be closed when doing telemedicine and documentation must be adequate and properly entered into the EHR. There is always a concern for hacking and breaching of data so firewall protection and security must be monitored. Stanford has their own portal for telemedicine visits. Since Patricia works for a large health care system like Stanford, things are constantly changing. She keeps up to date with new laws and regulations to ensure compliance by attending seminars and attending hospital leadership meetings. Apart from legal concerns, other shortcomings of telemedicine are the decline in quality of care. Some patients rather not schedule a telemedicine visit because they feel that they will not receive the same level of care. Not all health concerns can be assessed online. This directly affects the patient-physician relationship. Since telemedicine alters the context of the traditional face-to-face physician-patient trust-based relationship, Patients may not want to continue care with their physician if they feel that they aren’t getting the proper medical attention that an in-person visit would entail. They may not have faith and confidence in the healthcare system. It is crucial to educate patients who are concerned about the effectiveness of telemedicine services and prefers the traditional way of in-person visits by explaining policies and protocols and being empathetic with patients to provide quality care.
