The data would include the personal identifying information such as name, date of birth, gender, address, and contact information. In addition, information on the location of the clinic and medical information including the vaccine dose that has been administered, when it was administered, and if there are other doses pending. Public health data for a mass inoculation program can be obtained at local, state, or federal level. Some local governments use a patient tracking system called personal digital assistant (PDA) (Institute of Medicine, 2010). This system works by scanning the driver’s license of patients and creating a mass clinic. It is an effective data collection system that can be used for a public health emergency response. At a federal level, the Center for Disease Control and Prevention (CDC) is a great source to collect data from. The CDC uses a very efficient way to collect data by conducting phone surveys with immunization program managers (Institute of Medicine, 2010). During a public health emergency response, healthcare professionals can obtain data from the website of CDC.
