Mrs. Helen Stanley is an 84-year-old woman with current osteoporosis, lumbar compression fractures, and chronic pain.  She also has Giant Cell Arteritis (GCA)/Polymayalgia rheumatica (PMR)—treated with steroids since 2001—and a colo-vesticular fistula since 2002.  She also has diabetes, COPD and hypothyroidism, all controlled with multiple medications.  She also suffers from notable depression since the death of her spouse 5 years ago.

Mrs. Stanley lives in a second-floor apartment by herself, and all three of her children have passed on. Â She does not drive and is socially isolated. Â Her primary insurance is Medicare.
Mrs. Stanley has an extensive history of hospitalization. Â In the past five years, she has been hospitalized for hypotension, congestive heart failure, severe UTI, pain management due to compression fractures in her lower back, symptomatic uterine prolapse (spontaneously resolved), and COPD exacerbation.Â
Two months ago, Mrs. Stanley had minor trauma caused by sitting down abruptly on the commode. Â This resulted in excruciating low back pain the following day. Â She was hospitalized for pain control, and no new fractures were found. Â She was discharged to home.
Unfortunately, her functional status steadily worsened along with her back pain, and she was readmitted to the hospital for pain management and consideration of minimally invasive spinal surgery  (vertebroplasty).  She experienced delirium and respiratory depression on morphine. A fentanyl PCA/patch was tried with some relief. After extensive discussion with the radiologist, it was decided that the risk of sedation outweighed the potential benefit of the procedure, and she received an epidural steroid injection instead. While her pain improved, no functional benefits were noted (for example, she is unable to sit on the toilet for a BM without severe pain).  While in the hospital, she reported pain in her vulvar/labial region and experienced vaginal bleeding. She is currently incontinent. She also experienced a notable exacerbation in her CHF and COPD symptoms, including such severe dizziness, weakness, and shortness of breath that she cannot walk independently. She is also unable to stand long enough (or safely enough) to prepare food.  Mrs. Stanley’s depression has also steadily increased.  She frequently and tearfully expresses her strong desire to return home.  She misses her own bed, her cat, and her own things around her.  She desires her plan to be discharge to home.
Currently, Mrs. Stanley is as medically stable as the medical staff can make her in the hospital. Â She will be discharged from the hospital tomorrow.

I would go around to the local High Schools and talk to the teenage girls. I would let them know where they can go to get help if they were to become pregnant and the importance of getting prenatal care. I would also set up a health fair with booths from various organizations within the community that can give additional information about pregnancy and the effects of low birth weight in infants. They would have reading material as well that they can take home and read at a later time. I would also offer health fairs several times a year to constantly educate our community. Student #2: I believe that the first actions needed are to assess the circumstances of the low birth weight and to provide the correct education to new and expecting mothers and families. A large portion of health related incidents occur due to lack of health education. It is very important that the public is well educated in caring for infants and for themselves with pre-natal care. They should also be provided information on infant mortality and SIDS in order to bring awareness and provide prevention methods. Some community partners to help with this campaign could include Planned Parenthood, the county health department, and other community health centers. The main way to begin moving the action forward within the community is to research on prevention of mortality and improving birth outcomes. According to the CDC’s Division of Reproductive Health, improved birth outcomes and lower rates if infant mortality are done with improvements to perinatal care, improvement in the knowledge of SIDS and SUID, and conducting research to support the health of mothers and infants (CDC, Sept. 2020)

Remember how you made a list of observations about all the production elements: lights, sound, media, acting, costumes, directing, props, staging, set design? Keep that in mind when you watch your second performance. Take a notebook with you to the show and write down what you see. This will help you write the essay. PLEASE NOTE: This assignment is different from Performance Response 1. This assignment is an essay with a thesis, supporting paragraphs, and a conclusion. The Golden Rule for these essays is to point out details over generalities. Your response should use the observations you made and recall to support an argument (thesis). Your thesis is a one sentence statement that tells what you think the show was about. I am not interested in whether you liked the performance or not, or even whether you thought it was good or not — I want you to explore what you thought the production was trying to do and how it went about doing it. You do not need to cover every aspect of the production; as you think and write, focus on those things that are most helpful in supporting your argument. Remember that it is not enough to say that, “the costumes helped define the characters.” You must be specific. “The use of a stiff, canvas fabric for the clergyman’s costume helped create him as a rigid and inflexible character. This is another example of how the production was trying to depict the church as insufferable and organized religion in general as a great evil in the world.â€So when you watch your play think about what the “message” of the play is. In other words, if you see a production about a man whose hate for someone is controlling every part of his life, (he can’t sleep, he loses his job, he neglects his wife and children to the point of familial disaster) maybe your thesis statement would be, if you allow hate to control your life it will destroy you. So what is the lighting doing that conveys that? Does it grow darker? Does it flash when he’s wrestling with the hate? Does it shift showing separation between the man and everyone else? What about the costumes? Does the man’s white, button down shirt become torn at the sleeves and yellowed under the armpits? Does he lose his shirt? What about the staging? The set? The sound? How does the actor’s behavior change? Does he carry himself differently? Does he develop a limp? Again the Golden Rule for these essays is to point out details over generalities. Remember: you may end up writing (or re-writing) the introduction only after you have developed your argument (thesis), but whenever you actually write it, the introduction goes in the first paragraph and that paragraph should end with your thesis statement. Do not forget to write a concluding paragraph at the end of your paper. Remember, also, that paragraphs generally focus on one main idea– re-read what you write and add paragraphs where necessary. Finally, make sure that your sentences progress logically from one to another. Avoid sentences such as, “The play used costumes very well. The use of multiple levels in the set was exciting. I thought the red dress worked the best.” These sentences are too general and don’t convey enough meaning.One last time, the Golden Rule for these essays is to point out details over generalities.
