For this week’s discussion, I chose to focus on my current employer, Crystal Run Healthcare. Crystal Run Healthcare was presented with the opportunity to merge with a large health system out of New York City, Monetfiore. This merger would allow Crystal Run Healthcare the opportunity to provide cutting-edge care to their patients that they cannot get locally. Leaders of both health systems stated that the merger is an efficient way for Crystal Run to work with a prestigious research institution to deliver advanced care that is unavailable locally (Axelrod, 2018). Even though the merger was meant to be a great opportunity, it had conflicting effects on Crystal Run Healthcare.
           The merger negatively effected Crystal Run Healthcare because seven doctors filed lawsuits against the organization in regards to the merger. Crystal Run physicians claim that Crystal Run patients seeing Montefiore doctors is one of the deal’s problems (Axelrod, 2018). The merger would require Crystal Run doctors to refer all patients to Montefiore even if the physician does not believe such a referral to be in the patient’s best interest (Axelrod, 2018). Tensions rose between doctors at Crystal Run because the merger also stated that physicians had to provide three years’ notice before leaving the practice. Other health systems weighed in on the merger and stated that the treatment of Crystal Run physicians under the deal was disrespectful and potentially unethical. The merger led to a gray cloud floating over Crystal Run and tensions between physicians, and ultimately the merger did not go through. Instead, Montefiore purchased a 33 percent interest in a Crystal Run holding company (Axelrod, 2018).
           If the merger were to go through, I think many doctors at Crystal Run would be angered at the new regulations that the deal would have put into place. I also think Crystal Run patients would receive lower quality care. The merger would have required Crystal Run physicians to refer all patients to Montefiore health system locations, most of which are outside of the area. Patients would be forced to travel to the referred location even if it was not in the patient’s best interest.

My former employer was Hollywood Presbyterian Medical Center (HPMC) in Los Angeles and it really was a great organization to start my nursing career. I had the opportunity to do a new grad program at HPMC in the Telemetry department and it was quite the learning experience that opened many doors for me. HPMC is teaching hospital and I learned that firsthand during my new grad program. I knew how valuable that new grad program was and I took every learning opportunity I could. Before I was selected into HPMC’s new grad program, I was a nursing student there during my BSN clinicals. Being a nursing student there, I had the privilege to see how the organization worked and how the nurses were treated and the patients. I remember being in awe of how the staff was so supportive of each other and always teaching and giving advice to one another.

As a nursing student, I had an amazing nurse to shadow and I really made a good connection with the nurse and made an effort to create a good professional relationship. In creating that professional relationship, I had good references in applying to the new grad program after I graduated with my BSN. From the charge nurse to the supervisor, they were all very approachable and always making sure we were helped when needed and very supportive. “Our dedicated team of healthcare professionals works together to plan and coordinate every level of your care. We want to make sure you understand your care plan and feel comfortable, that’s why we ask that you communicate with your nurses, physicians and other members of your care team” (Hollywood Presbyterian Medical Center, 2021). HPMC’s effect on myself and other employees there was proof that it was adding positive value to the organization through its way of treating employees and patients.

According to Langebeer & Helton (2016), it has been suggested that the chance of a complex project surviving and achieving all of the benefits it established early on is around 50%. Successfully managing a project from start to finish requires certain key skills (Joubert, 2020). The two risks that I perceive to be the greatest threat to successful project completion are poor communication and lack of formalized documentation or procedures.

           Poor communication poses a great threat to successful project completion because without everyone being on the same page, no one is working towards the same end goal. Without strong communication skills, project managers would find it incredibly difficult, if not nearly impossible, to effectively manage their teams and coordinate efforts in order to bring about a project’s successful resolution (Joubert, 2020). Think about doing a group project for school. If there is poor communication between the group, parts of the assignment may not get done or parts may be duplicated, and the project would not be successfully completed. This is what would happen in the healthcare industry without proper communication, just on a much larger scale.
           The lack of formalized documentation is also a great threat to the successful completion of a project. Documentation must lay the foundation for quality, traceability, and history for both the individual document and for the complete project documentation (Verma, 2021). Formalized documentation allows for the project to move forward at a speedy pace and ensures that all stakeholders are as informed as possible (Verma, 2021). Having formalized documentation helps all members of the project remember the goals and objectives at all phases of the project. If an issue were to arise during a project, formalized documentation would provide traceable data back to where, who, or how the issue originated.

I have chosen these two risks that I perceive to be the greatest threat to successful project completion: poor communication and lack of financial resources.

Poor communication: In my opinion, communication is the key to moving forward in any type of relationship. This includes work relationships where the leader is trying to meet a goal with their subordinates. In this case, that could apply to a project manager trying to reach the finish line with the employees involved to complete the project. A way a healthcare administrator could combat this is by creating routine meetings with direct reports. The meetings should be at least weekly and have structure to them, such as an agenda with the expectations of the meeting given beforehand. “Continuous communication also helps to reinforce the concept that the project is important and that there is no hidden motive or purpose” (Langabeer & Helton, 2016).
Lack of financial resources: I chose this threat as one of the most impactful because if the project isn’t supported with enough money, the project will undoubtedly fail or never finish. Like communication, sufficient funds keep the project moving forward toward completion. To keep the funds secured, a good idea is a sponsorship and promotion. “Projects require sponsorship to secure financing approval and to ensure the commitment of the right people on the project from the outset” (Langabeer & Helton, pg. 182, 2016). Another idea could be to cushion the project with extra funds. Meaning have the project cost less than the budget allotted or reserve more money than the cost of the project. Excellent project managers need to leave room for error, as this will most likely happen.  
According to the textbook, there is a problem within hospitals centered around power (Langabeer & Helton, 2016). This is no surprise, as most businesses have similar issues when management comes into play. For example, physicians are an example of a general department that clashes with the administration department. Seeing as physicians are the ones who have “medical expertise and control of the customer,” they often act entitled to influence the decision of the project manager or at least make sure their voice is heard (Langabeer & Helton, 2016). A department well suited for handling issues such as this is the Human Resources department. They are the “lawyers” within the hospital, making sure the employees’ voices are heard as well as their managers without crossing an unprofessional line, or even worse—a harassment line.

Staff-to-resident abuse in nursing homes is a complex and multifaceted phenomenon associated with multiple factors on different levels of the ecological model. This study aimed to explore individual, relational, and institutional characteristics associated with perpetrated staff-to-resident abuse in nursing homes” (Botngard, 2021). I found this article interesting. Within my local community/area, we have seen more financial abuse within elderly population than physical, mental, and/or emotional. The study looked at general information such as gender, age and education status. They then asked questions about mental health such as harming yourself, feeling exhausted and then moved to childhood and currently feelings about work environment. They also asked questions about the residents and workload. The point of the questions is to find out the risk of elderly abuse. The study reported “This study of risk factors associated with staff-to resident abuse in Norwegian nursing homes showed that various factors in the ecological model increase the likelihood of staff perpetrating psychological abuse, physical abuse, and neglect. The predictors most strongly found to be associated with all three types of abuse were 1) being a registered nurse/social educator or licensed practical nurse, 2) reporting symptoms of psychological distress, 3) considering leaving the job, 4) reporting poor attitudes towards persons with dementia, 5) and experiencing care-related conflicts and resident aggression. Other predictors were poor quality of childhood (neglect) and lack of support from a manager (psychological abuse)” (Botngard, 2021).Within this study continuing education was stated as more no’s than yes in the survey. This makes me question if a good prevention to reduce elder abuse in all forms is to require a course every year to reduce the risk. This would be good for all macro, mezzo and micro levels. Macro level should require it such as the state. Then Mezzo the facility or community needs to require it. Which then helps the micro level reduce the risk. Another, invention/prevention, I have been thinking about. As stated above, in my local community/area, we have had many reports and criminal charges for elder abuse for financial abuse. Elder financial abuse is a type of elder abuse in which misappropriation of financial resources or abusive use of financial control, in the context of a relationship where there is an expectation of trust, causes harm to an older. My thought for the prevention of this, is the make people away of these issues and how to report. Many of the local cases there was someone who noticed it but did not say anything due to lack of evidence, lack of education on how to report, and no one person should micro-manage the money. Example: the most recent financial abuse that was criminally charged, was the group home house supervisor was the only one to handle financial transactions. This should be a red flag for everyone and anyone involved because someone even if it is one or two other people in a different position double check and triple check the transaction and bank statements to hold everyone accountable. Assessment Instructions
Record a slide presentation, with audio voiceover, for one of the stakeholder groups you identified in your Assessment 2 Policy Proposal. Inform the group of current performance shortfalls, introduce the proposed policy, explain why the policy is needed, and present policy-driven practice guidelines to resolve the performance issue. You must also obtain buy-in from the group by explaining the positive effects of the policy and practice guidelines on their work.

The Coca-Cola corporation is organized around a board of directors that the shareholders appoint. They majorly decide and come up with final decisions regarding the company’s operations. The board of directors’ decisions and initiatives are carried out by senior management and various committee members. The board of directors of the Coca-Cola Company is entirely responsible to its shareholders (Nsomba, 2021, p. 2). The committees evaluate themselves and report back to the board of directors. The CEO’s annual self-evaluation and appraisal are handled by the board of directors (Nsomba, 2021, p. 4). Aside from the board of directors, the senior operations leadership members are in charge of the different CocaCola firms globally.

James Quincey is the Chief Executive Officer of the Company. Quincey is driving Coca-Cola’s transformation into a whole beverage corporation. The CEO is responsible for the overall performance and success of a firm. They may seek counsel on essential issues, but they have the last word in decision-making. The Chief Financial Officer of the CocaCola Company is John Murphy. He oversees the firm’s worldwide finance operation and presents the firm to various stakeholders in this capacity. The Chief Operating Officer is Brian Smith. He plans and implements corporate functions, establishes policies that promote company values and purpose, and oversees company operations and administrative work as a Chief Operating Officer.
Importance of the Organization and Leadership
The ability to lead a global firm towards achieving an organization’s mission is organizational leadership. The success of every company is primarily determined by its leadership. No company can function efficiently without proper oversight and guidance (Pendergrast & Crawford, 2020, p. 11). A company is established to achieve specific goals through a team of skilled individuals; it is then critical to control the qualified individuals for things to go according to plan. While an entity may contain various skills and abilities, employees, leadership is responsible for directing individual efforts toward a common objective (Pendergrast & Crawford, 2020, p. 12). Influential leaders clarify the organization’s goals and objectives, motivate employees, and assist them in achieving them. Leaders help their firms achieve success by empowering employees and coordinating personal efforts to develop a common goal.

2-Hello Class, the organization that I would like to build would start with the Owner and president which would be myself. Then there would be an onsite supervisor. Next, there would be lead people in every department of the company. The organizational structure is very important to this organization. The business structure determines how well the organization is operated. Structure determines how clear and effective the operation plans are executed. This determines how smooth the organization will be operated.
Leadership structure can determine the success of the organization. This is one of the main crucial elements that impacts the development of structure and employee success. Transformational leadership can be described as a way of leadership where a leader cooperates with groups of employees to understand desired alterations, making a vision to preside over the alteration through motivation, and enforcing the alteration in arrangement with dedicated individuals of a group. Transformational leadership has been deemed to be the aim of interest to the researchers due to its important and great effects on organizational and also employee success. 
Leadership structure is a critical element that influences the advancement of organizational success and employee’s job performance, and determines the goals that are desired, which also creates a revenue for their workers or creates another social and financial addition to the community. Employees’ work success advances by increasing work satisfaction and job dedication. The main focus of any institution is to optimize workers’ job success; therefore, a business may maintain with their rivals.

3-Microsoft Corporation’s organizational structure has set divisions into engineering groups and business functions. Each type of product or service has its own research and development, sales and support. The engineering group is further divided into Cloud and AI, Experiences and devices, and Artificial intelligence. On the other hand, the executive branch is divided into business development, corporate external and legal affairs, corporate strategy and planning, finance, global sales marketing and operation, HR, Linked In, marketing group, and worldwide commercial business (Dudovskiy, 2019). Even though each division has some autonomy to make decisions, they still must report to the current CEO Satya Nadella. Compared to other organizations, like Apple for example, that works on a very decentralized system, Microsoft has had to keep in some centralization aspects (Harisai & Lim, 2017). This is because as many different departments expand it is difficult to keep the same rules and values across all departments.
The structure of the organization guides the type of culture the organization will have as well. For example, when Microsoft was founded, it was originally a very centralized organization, eventually, they realized it was not appropriate in the long run as their employees were starting to become unhappy due to being ruled and controlled by one person (Harisai & Lim, 2017). Issues arise as the company expands as it would become time-consuming and not efficient to have only one person approving all moves. If this is the case, a company this large would not quickly be able to make decisions. Decentralization occurs where individuals independently make their own decisions, trying to optimize their own objectives (Marques, Agostinho, Zacharewicz & Jardim-Gonçalvez, 2019). This progresses onto the culture of the company because if the business is operated well, and employees feel satisfied then, the company will have a positive environment all around. 
The company’s success does not only depend on having a superior product or service to satisfy the customer; employees are just as important to satisfy. Research shows that having a conductive and embracing environment at the workplace encourages and enables employees to express their ideas, participate in decision-making process and allow their skills and talents to flourish through collaboration and uniformity (Isac, Dobrin, Raphalalani, & Sonko, 2021). Leadership is a big part of organizational culture as well as they should exude the organization’s vision and mission to reflect how the work environment should be. In the case of Microsoft, the business plan should also reflect their current culture and align with the CEO’s vision for the company for it to be successful.

As you begin to consider the assessment, it would be an excellent choice to complete the Breach of Protected Health Information (PHI) activity. The will support your success with the assessment by creating the opportunity for you to test your knowledge of potential privacy, security, and confidentiality violations of protected health information. The activity is not graded and counts towards course engagement.
Health professionals today are increasingly accountable for the use of protected health information (PHI). Various government and regulatory agencies promote and support privacy and security through a variety of activities. Examples include:

Meaningful use of electronic health records (EHR).
Provision of EHR incentive programs through Medicare and Medicaid.
Enforcement of the Health Insurance Portability and Accountability Act (HIPAA) rules.
Release of educational resources and tools to help providers and hospitals address privacy, security, and confidentiality risks in their practices.

Technological advances, such as the use of social media platforms and applications for patient progress tracking and communication, have provided more access to health information and improved communication between care providers and patients.
At the same time, advances such as these have resulted in more risk for protecting PHI. Nurses typically receive annual training on protecting patient information in their everyday practice. This training usually emphasizes privacy, security, and confidentiality best practices such as:

Keeping passwords secure.
Logging out of public computers.
Sharing patient information only with those directly providing care or who have been granted permission to receive this information.

Today, one of the major risks associated with privacy and confidentiality of patient identity and data relates to social media. Many nurses and other health care providers place themselves at risk when they use social media or other electronic communication systems inappropriately. For example, a Texas nurse was recently terminated for posting patient vaccination information on Facebook. In another case, a New York nurse was terminated for posting an insensitive emergency department photo on her Instagram account.
Health care providers today must develop their skills in mitigating risks to their patients and themselves related to patient information. At the same time, they need to be able distinguish between effective and ineffective uses of social media in health care.
This assessment will require you to develop a staff update for the interprofessional team to encourage team members to protect the privacy, confidentiality, and security of patient information.

Preparation
To successfully prepare to complete this assessment, complete the following:

Review the infographics on protecting PHI provided in the resources for this assessment, or find other infographics to review. These infographics serve as examples of how to succinctly summarize evidence-based information.

Analyze these infographics and distill them into five or six principles of what makes them effective. As you design your interprofessional staff update, apply these principles. Note: In a staff update, you will not have all the images and graphics that an infographic might contain. Instead, focus your analysis on what makes the messaging effective.

Conduct independent research on the topic you have selected in addition to reviewing the suggested resources for this assessment. Instructions
In this assessment, assume you are a nurse in an acute care, community, school, nursing home, or other health care setting. Before your shift begins, you scroll through Facebook and notice that a coworker has posted a photo of herself and a patient on Facebook. The post states, “I am so happy Jane is feeling better. She is just the best patient I’ve ever had, and I am excited that she is on the road to recovery.”
You have recently completed your annual continuing education requirements at work and realize this is a breach of your organization’s social media policy. Your organization requires employees to immediately report such breaches to the privacy officer to ensure the post is removed immediately and that the nurse responsible receives appropriate corrective action.
You follow appropriate organizational protocols and report the breach to the privacy officer. The privacy officer takes swift action to remove the post. Due to the severity of the breach, the organization terminates the nurse.
Based on this incident’s severity, your organization has established a task force with two main goals:

Educate staff on HIPAA and appropriate social media use in health care.
