Posted 1 Two circumstances that an individual’s return has a greater chance of being audited are: Erroneous data entry Unusual/Unrealistic deductions Erroneous data occurs when individuals are carelessly inputting their information on their return and/or not waiting until they have received all of their documents (TurboTax, 2021). The latter part happened to me this year. I totally forgot about my 1098-T form which resulted in me filing my taxes without it; I had to amend my return. This can be avoided by carefully going back over the return before it is filed. I would suggest triple checking the return and if one is able to have someone review the return as well. Also, a good practice is to wait until you receive all documentation before filing (lesson learned for me). Unusual/unrealistic itemized deductions are tailored towards individuals and small business owners (TurboTax, 2021). For small business owners, their business expenses must be both ordinary and necessary. For individuals, daily commuting to your regular job is not deemed as a deduction (TurboTax, 2021). This can be avoided by looking on the IRS website to see what qualifies as deductions. Hiring a CPA or tax specialist to discuss tax situation and review deductions before filing return. The CPA or tax specialist will be able to guide the taxpayer on deductions that best fit their situation, if any are applicable. Posted 2The first circumstance that will increase an individual’s chance of audit is failure to report dividend income and or distributions from an individual retirement account (Iacurci, 2020). There has been an increase in third parties reporting income paid out to individuals to the IRS along with the taxpayer annual return. The IRS uses computers to compare what has been reported on the taxpayers 1040 return vs the information sent to them from financial institutions and other sources of information. Also, when an omission is found by the IRS, interest and the accuracy penalty of 20% of the underpayment of tax is normally added and collected. The best way to avoid this circumstance is to make sure you are reporting all your income on your tax return. The second circumstance that will increase an individual’s chance of an IRS audit is if you take deductions that are not in line with your income. The way to avoid this in an audit is to make sure that your deductions are documented, and you can provide proof of the deduction in case of an audit. The IRS will compare your deductions from year to year as well in order to determine if an audit is due on your tax return(s) as well (Fishman, 2020). As the IRS’s budget continues to be decreased by Congress each year and the audit rate continues to decrease due to staffing limitation, the IRS has increased the sophistication of the software they use to identify issues with tax filings. For example, in 2019 closed over 364,000 cases under its Automated Substitute for Return program which resulted in over six billion in additional tax income (Fishman, 2020). This program uses data the IRS receives from various sources and then compares its filings database to verify a return was filed. These services cannot be provided by the outside auditing firm, and should be avoided. A full list of ethical rulings on other relationships which might compromise auditor independence, integrity, or objectivity is also listed on the site (PCAOB, ET Section 191, 2003-2021). Rule 3523 specifies that the auditor can also not perform tax services for people in Financial Reporting Oversight Roles, unless the person’s role is derived from serving as a member of the board of directors (or similar oversight body), the person in the role is an unrelated affiliate whose financial statements are either not material, or are prepared by a different auditor, or the person was not previously in the role, except for a promotion, hiring, or other change in employment, and the tax services were provided previously, and completed no more than 180 days after the hiring or promotion (PCAOB, Section 3, 2003-2021). Under Rule 3524, the role of the audit committee is to pre-approve certain tax services which may be provided by the auditor, which would not compromise the independence, integrity, or objectivity of the auditor (PCAOB, Section 3, 2003-2021). The public accounting firm would be required to describe, in writing to the audit committee, the scope of services, the fee structure, as well as any amendments to the engagement letter, between the firm and the audit client, as well as disclosing any referral fee/agreement/fee-sharing arrangement between any person rendering services other than the audit client and the public accounting firm (PCAOB, Section 3, 2003-2021). The public accounting firm must also discuss with the audit committee the potential impact the services provided might have on independence, in addition to documenting the substance of the discussion with the audit committee (PCAOB, Section 3, 2003-2021). As listed in Rule 3501, “Definitions of Terms Employed in Section 3, Part 5 of the Rules,” subsection (a)(v), the audit committee is a committee or equivalent body, established by the board of directors of an organization, or (if no such committee exists), the board of directors themselves, or (if there is no board of directors), the person responsible for the oversight of the accounting and financial reporting processes and audits of the organization (PCAOB, Section 3, 2003-2021). The audit committee’s role is to ensure that the interests of various stakeholders are served, and that the additional services (tax or otherwise) do not compromise the ability of a public accounting firm to provide audit or attestation services with objectivity and independence, so that the integrity of these services is not compromised. Without these safeguards in place, the public trust can be undermined when auditing firms (who provide additional services) become complicit with their auditing clients who operate unethically or illegally, as was the case with Arthur Andersen and Enron. The audit committee serves as a safeguard, designed to prevent similar accounting scandals from happening in the future.

Introduction
As a baccalaureate-prepared nurse, you’ll be positioned to maximize the use of technology to achieve positive patient outcomes and improve organizational effectiveness. Providing holistic coordination of patient care across the entire health care continuum and leveraging community resource services can lead both to positive patient outcomes and to organizational improvements.
Preparation
In this assessment, you’ll determine how health care technology, coordination of care, and community resources can be applied to address the health problem you’ve defined. Plan to spend at least 2 direct practicum hours working with the same patient, family, or group. During this time, you may also choose to consult with subject matter and industry experts.

It is also what makes population health monitoring possible, and that type of metric is what accountable care organizations (ACOs) and incentive programs like Meaningful Use and initiatives from the Centers for Medicare and Medicaid Services (CMS) are based on. At the state level, efforts to expand and increase the exchange of health information is funded by the State HIE Cooperative Agreement Program (healthit.gov, n.d.). In March of 2010, all states in the union and eligible territories received award amounts that were based on population. Although more than half of all office-based physicians were already using EHRs by that time (Deas & Solomon, 2012), this funding encouraged the rest to come onboard and for all parties to strive for Meaningful Use achievements.Implementation and Adoption As part of the HIE Cooperative Agreement Program, Pennsylvania’s Governor’s Office of Health Care Reform received an award of $17,140,466 (healthit.gov, n.d.). As the state-designated entity, this office was then tasked with using these funds to develop technical services, governance, policies, and operational mechanisms for increase health information exchange. One of the most significant ways that the state of Pennsylvania is encouraging the expansion of HIE is through its PA REACH program. This program created two regional extension centers, one in eastern Pennsylvania and one in Western Pennsylvania, to provide assistance as needed to physicians and practices venturing into the world of electronic health records (EHRs) and attempting to qualify for Meaningful Use benchmarks. As the previously cited authors state, “an HIE organization does not establish itself as a neutral, trusted entity overnight” (Deas & Solomon, 2012, p. 167). PA REACH, however, has achieved this level of trustworthiness by providing reliable assistance across the state.Two specific “success stories” can be found on the healthit.gov (n.d.) website which describe practices that accessed assistance from the PA REACH eastern branch and were able to meet the Stage 1 requirements for Meaningful Use within their first year of EHR usage. The first example is a solo practitioner in East Stroudsburg, Pennsylvania. This physician went into EHR adoption expecting the worst but ended up pleasantly surprised thanks to the ever-present support from his PA REACH project coordinator. The second success story was from a hospital that belongs to the health system of this post’s author, St. Christopher’s Hospital for Children in Philadelphia. The need for cultural and workflow changes were major challenges during initial EHR adoption at St. Christopher’s, but the PA REACH team helped staff learn to customize reporting structures within their selected EHR to meet their specific needs. Although both examples resulted in the ultimate success of reaching Meaningful Use Stage 1, there were additional successes and lessons learned. At St. Christopher’s, new functions of the EHR resulted in reduced wait times by fifteen minutes per patient and a fourteen percent reduction in no-show rates. For the solo practitioner, his office receives less questions about billing post-EHR implementation, although he was unable to maintain normal patient volumes during the “go-live” phase of implementation due to the learning curve for staff.Biblical Integration and Conclusion In conclusion, the establishment of two well-respected regional extension centers in the state of Pennsylvania have played a significant part in widespread EHR adoption and Meaningful Use achievements. Studies have shown that in states where regional extension centers were not created until much later, practices are more likely to fail Meaningful Use audits (Lehmann et al., 2018). These authors provide the example of practices in Georgia which often failed the security risk assessment portion of the audit due to a lack of training that, for other states, came from the regional extension centers which remained nonexistent in Georgia. The importance of the establishment of regional extension centers as part of the HIE Cooperative Agreement Program cannot be understated. Per the Office of the National Coordinator for Health IT, “regional extension centers assisted more than 147,000 clinicians through 2013, with more than 85% of those providers live on an EHR” (Shanholtzer & Ozanich, 2016, p. 70). Physicians and staff were largely scared and apprehensive about the massive changes required during EHR transitions, but these programs eased the pain. Investing funds into helping others, especially during times of stress, is a biblical concept. Hebrews 6:10 reminds Christians that God “will not forget your work and the love you have shown Him as you have helped His people and continue to help them” (Holy Bible, New International Version, 1978/2011). The week four discussion board prompt question has asked the students of the class to examine a very common question when it comes to the topic of health care within the United States. That question has often centered around the competing models of for-profit and not-profit health care organizations, and which model happens to be superior. The week four question has asked the students of the class to directly compare and contrast the benefits of not-profit versus for-profit healthcare organizations. This author will attempt to provide at least two benefits of each particular model in the following passages. This author will select to discuss the benefits of not-profit health care organizations first, as they make up the majority of health care institutions in the United States (Cronin et al., 2021).
As this author has already pointed out, the majority of hospitals within the United States happened to be not-profit, and the driving reason behind that occurrence has been simple and economically driven (Cronin et al., 2021). Arguably, the reason why the majority of hospitals happen to be not-profit in the United States has been because of the tax-exempt status which they have received from the Federal Government. The benefit of the tax-exempt status has meant that not-profit health care organizations have been able to achieve lower average costs which they can pass onto their patrons (Lee, 2019). Additionally, not-profit health care organizations benefit in a public relation sense, which potentially draws more sales, because the perception of not-profit has been viewed in society as altruistic despite in many instances not-profits having brought in the same or better revenue than for-profit institutions (Jeurissen et al., 2021; Lee, 2019). With these two points being made for the benefits of not-profit health care organizations, what can be said of the benefits of for-profit health care organizations?
Perhaps, the two most compelling benefits found with the for-profit health care organization model has been related to the more definable top-down organizational structure which as lent it to having superior efficiency/operational discipline and agency (Lee, 2019). Agency has benefited for-profit organizations because it has tied and incentivized key members and leaders of the organization to mutually align their own interests, or the interests of the board, with those of the health care organization (Cronin et al., 2021; Lee, 2019). In turn, this has meant that for-profit hospitals have been able to charge more for services and focus on narrow fields of care due to not having to reinvest the majority of their revenue back into the mission of the institution (Jeurissen et al., 2021). These factors have meant that the for-profit institution can be efficient and well defined without superfluous personnel and mission creep into loosely relevant areas which consume more capital.
In closing, this author has stated earlier that for-profit health care organizations often encounter negative stigma because of the contradictory relationship profit and compassion have with one another (Jeurissen et al., 2021). For this contradictory aspect, this author has selected 1 John 2:9, “Whoever says he is in the light and hates his brother is still in darkness.” (English Standard Version, n.d.) as his Biblical integration. This verse has captured the conundrum of saying that medicine is being practice for the benefit of others in the name of compassion and charity, but yet we charge large sums of money for what is supposed to be charity and compassion. Are we charitable and compassionate, or are we once again fooling ourselves into believing that we are in the light?

A non-profit and for-profit are two very important aspect in health care organization. Before comparing and contrasting the benefits of non-profit and for-profit health care organizations, it is important that the the concepts are first discussed so as to have a good understanding of how they both operates. The strategies of not-for-profit organizations may differ from those of for-profit organizations because of more severe agency problems, differences in goals, and differences in costs (Lee, 2019). Every company or organization have an agency issue, these agency problems are battles between what the manager wants for their own interest and the objectives or goals of shareholders or team members. In terms of funding, the non-profit health care organization gets their fund from charity, religious organizations, or educational funds. The overall goal is to provide quality care to all patients and this boils down to the real health care systems to utilize and administer quality care. Non-profit health care organizations have a significant role in the provision of health care, at least until 20 century and today, the role of non-profit health care organizations in health care varies significantly across health systems (Almeida, 2017). In the United States, about 60% of hospitals and about a third of nursing homes implemented or adopted the legal form of non-profit health care organization.
A simple way to define a non-profit organization is to first define the for-profit health care organizations. A for-profit health care organization will seek to encourages efficient production among the for-profit hospitals, while non-profit health care organizations might be less vigilant in eliminating waste in the production process (Freeman et al., 2020). There are external mechanisms for a for-profit organization to become more efficient such as selling of shares over dissatisfaction of managerial performance.
Looking at the differences between the non-profit versus for-profit health care organization, it should be noted that, since nom-profit health care organization lack incentive to maximize profits, non-profit health care organizations are expected to maximize prestige, quality, or quantity rather than strict cost. While on the other hand, the for-profit health care organization will encourages efficiency. The non-profit as already mentioned, received funding from charity organizations, religious organizations, and sometimes from government subsidies, while the for-profit health care organization’s sources of capital are the equity capital from the various investors and return-on-equity payments from third party payers like Medicare. Non-profit health care organization gain benefits from the pursuit of goals than the for-profit organizations, and in terms of costs, the non-profit organizations may not have to pay taxes which tends to make the non-profit’s average costs lower, while the for-profit pays taxes and therefore tend to have costs (Lee, 2019).
Although the non-profit health care organizations and the for-profit organizations have their differences, some similarities could also be seen from these two health care organizations. In both non-profit and for-profit health care organizations, employees share perceptions of the work environment in terms of encouraging organizational members to share and generate new ideas with one another (Brimhall, 2019). This has helped to foster a climate of innovation for both health care organizations. It must be noted that, whether it is a non-profit health care organization or a for-profit health care organization, it is essential for all health care organizations to meet patients’ needs and to provide quality of care to their patients. Providing the right care and quality health care to all patients should be the main goal for all health care organizations. Colossians 3:23 (New International Version) reads “And whatsoever you do, do it heartily, as to the Lord, and not unto men”. According to the scripture, it does not matter which type of health care organization is adopted, but it would be good to see that all patients are been given the right care they deserve.
