(CNN)With a dangerous Covid-19 variant on the rise, health experts are urging people who are still hesitant to get their vaccinations. But the US surgeon general warns a big obstacle stands in their way: Misinformation.
“There is so much misinformation out there about the vaccine, coming through so many channels — a lot of it being spread on social media,” Dr. Vivek Murthy told CNN’s Erin Burnett. “It’s inducing a lot of fear among people.”

“Two-thirds of those who are unvaccinated in polls say that they either believe the myths about Covid-19 or think that they might be true,” he added.Experts, including Dr. Anthony Fauci, have estimated that  (Links to an external site.)70 to 85% of people in the US will need to become immune to the virus through vaccination or infection in order to control community spread. But after initial surges, vaccination rates have slowed, and only 53.7% of the population has received at least one dose.
In addition, more than 1 in 10 people who have received one dose of the Pfizer/BioNTech or Moderna vaccine have missed their second dose, according to data shared with CNN by the US Centers for Disease Control and Prevention.
That statistic is especially concerning to experts because studies have shown that the vaccines are much more effective against the Delta variant after the two-dose series is completed.
And the Delta variant is believed to be more transmissible and cause more severe disease than other strains.
Murthy said he is worried for those who are unvaccinated as the variant spreads.
In Los Angeles County, the impact is already clear. Nearly all of the Covid-19 cases, hospitalizations, and deaths in Los Angeles County are occurring among people who are unvaccinated, county health officials said Thursday.
Out of nearly 437,000 positive coronavirus cases reported in L.A. County since December 2020, 99.6% of those were among individuals who were unvaccinated, health officials said in a press release.
“The virus is still with us,” Los Angeles County Public Health Director Barbara Ferrer said at a press conference. “Even now, we need to be careful to mask and maintain distance from people outside of our households, especially if they’re not yet vaccinated.”

Gateway drugs contribute to more powerful drugs but other factors may be involved. Marijuana, a gateway recreational drug, is legal but can serve to sensitize the developing brain to dopamine receptors and cravings (Panlilio et al). Interestingly, research shows that early adolescent drug use contributes to increased later adolescent drug use but, in adulthood, the correlation can adjust. At that point, mental health issues cloud the relationship (Nkansah-Amankra). Research shows that gateway drugs do contribute to later drug use but it is only one factor  in the complex relationships in addiction.

Imagine that your organization has a generous tuition reimbursement program, but currently employees must take classes outside of regular business hours. You’ve noticed that many classes you want or need at the local university are offered infrequently during evenings or weekends. Address your letter to the correct person or office.

2. Write a Formal Email
Write a Formal Email Persuading a Lender to Defer Paying a Student Loan (LO 2-2)
Many college graduates today leave school with large debts from student loans. Though the loans must be repaid, the time to do so and the monthly payment are determined by the lender. With a touchy economy making it harder to find a job, students may also find it harder to repay loans. Many lenders, however, expect them to start doing so upon graduation. You plan to request a deferral to start paying back your loan.
To do so, answer the following questions about your audience:

What is at stake for the lender if it defers the loan?
What regulating bodies (e.g., state and federal government) govern how a lender may alter the terms of a loan agreement?
What objections might a lender raise to deferring the loan?
What other resources might the lender suggest tapping in order to repay the loan?
What might the lender expect from you as a guarantee that you will repay the loan after the deferral?
What channel is the best way to reach the lender?
What tone will work best in the message to the lender?
How long of a deferral are you requesting?

Announcing Holiday Diversity (LO 13-1 to LO 13-3)
Your organization has traditionally given employees several holidays off: New Year’s; Martin Luther King, Jr., Day; Independence Day; Veterans’ Day; Thanksgiving; and Christmas. Employees who celebrate other holidays (e.g., Good Friday, Yom Kippur, Ramadan, Chinese New Year, the Hindu Holiday Diwali) have been able to take those days off with the consent of their supervisors. But some employees have complained that it is unfair to depend on the goodwill of supervisors. And now a few other employees have complained that people who honor other holidays are getting “extra” days off since they take those days in addition to the standard holidays. 
Therefore, the executive committee of your organization has decided to allow employees any 10 days off for holidays. They will have to tell their supervisors which days they plan to take off. People will be asked in December which holidays they want to take off in the following year. People can change their minds during the year as long as they have not yet taken off the full 10 holidays. Any religious, ethnic, or cultural holiday is acceptable. (Someone who wants to take off Cinco de Mayo or Bastille Day can do so.) Vacations, personal days off, and sick days are not affected by this policy. 
As vice president for human resources, write a memo to all employees, announcing the new policy.

response to SARA student one with 50 words ((((A romantic child as described by the readings has the love for and connected to nature and its living beings (humans and animals alike). A romantic child remains uncontaminated by the materialistic world. He/she is inherently good despite the lack of guidance from an adult or set of golden rules to follow. Kindness and compassion come naturally to a romantic child. These are the attributes possessed by Gluck. He demonstrated his kind heart through out the story. One instance – in letting in from the cold and rain an odd looking, strangely garbed 4 ft six in height strange gentleman despite his brothers’ instructions not to let anyone or give away anything with a promise of a beating if Gluck disobeyed. Gluck offered his own portion, (a slice of mutton) and fed this not only wet but hungry stranger. another instance-On his journey to the golden river for a prize of gold that awaits a deserving person, Gluck came across (in separate instances), an old man, a child, and a dog- who would perish if not given water to drink. Gluck gave each water to drink from his flask realizing there would be none left to drop unto the river thus eliminating his chance of attaining the gold as instructed by the King of the Golden River. This did not matter to Gluck as his main concern was to help the creatures who would otherwise die. This to him was more important than all the gold a river can give and saying “confound the king and his gold too,” Gluck poured all the water in his flask for the dog to drink. Unbeknownst to Gluck, these were tests by the King of the Golden River for a deserving man of the prize would be one who possess a kind heart; one who place other’s needs first before one’s own. Gluck’s brothers who ventured the same journey before him and presented the same circumstances failed the test and were turned to stones. In the end, Gluck’s reward was not of gold pieces, but the flow of the river created streams fertilizing the valley where his farmland was. Its rich soil brough forth ample plantings and crops. Gluck unlike his brothers never turned away a neighbor in need and he prospered for the rest of his life.
Gluck’s demeanor endeared him to the readers ( a success in Ruskin’s part in appealing to the readers’ emotions). What strikes me most was his capacity for forgiveness. He did not bear animosity nor feel vengeful towards his brothers despite the many beatings and maltreatment he received. Instead , he showed mercy. He showed his concern over his brother’s incarceration and was determined to get him out of jail. He gained employment so he could save money and pay the bail money for his jailed brother. Sam response with 50 words ((((The vibe I get from first reading this I thought the Old gentleman was scamming Gluck, which made me emphasis with the character that he was being played but he was just trying to be nice to the Old gentleman who came to his house. Gluck is stated to be the complete opposite of Hans and Schwartz by being kind and caring for others. He lets the old gentleman into his home because he is wet and give him his share of the food because he is hungry. As the reader we are already on Gluck’s side because of his actions to others. We can see Gluck as being admirable to the old man, child, and “dog” (really the King) that he passed on the way up the mountain. I think it’s also important to note that after he did these actions he was happy about it and didn’t think twice about what he felt he needed to do. After the child, he continued to climb and never felt “so happy in his life” because he saw beautiful flowers after passing the child. We know that Hans and Schwartz never saw this. This points out a lesson saying if you help others you can be rewarded without asking. Gluck was given joy and he wanted joy for he has worked hard and been tormented long enough. Gluck is also seen as admirable by consistently standing with his brothers even though they are terrible to him. When Hans did not come back from the mountain Gluck was scared for him that he cried at night. He then took the initiative to get Schwartz out of jail by working hard and making money and food for them. Schwartz repays him by stealing this money and going to the mountain himself. Gluck would be an example of “The Child who is instinctively virtuous and not in need of formal education or sets of rules” because he never was told how to act he was just naturally a more caring and better person than his brothers. I think Gluck lives through his emotions in a sense that he makes decisions and goes with them based on how he feels. For example, he didn’t want to give the dog a drink a first but then he decided he would because his emotions got to him and made him feel bad for the poor animal. Hans and Schwartz don’t live off their emotions and just do what will be best for them and not helping others. Hans and Schwartz both noticed the thirsty people and dog and felt bad but not bad enough to where it would change their mind to help. Living and feeling your emotions and having them make decisions can be a good thing because Gluck was the most generous character. Although, this can also mean that you can be talked into anything and used and his brother definitely used him based on his emotional character.)))))

COPD is defined by the Global Initiative for Chronic Obstructive Lung disease (COLD) as a common preventable and treatable disease that is characterized as persistent respiratory symptoms and airflow limitation. This limitation is due to airway and/or alveolar abnormalities usually caused by significant exposure to noxious particles or gases, such as cigarette smoking (Kennedy-Malone et al., 2019).      
What additional subjective information will you be asking of the patient? 
Because COPD is a complex, chronic disease, patients require a comprehensive assessment of their symptoms. As the healthcare provider, I would ask him detailed questions regarding symptoms of dyspnea, cough, sputum production, lower respiratory infections, chest tightness, and fatigue. In the collection of data regarding history of present illness (HPI) I’d have the patient explain the onset, location/radiation, duration, characteristics, aggravating and alleviating factors, timing and severity of his symptoms. I would question what medications he is currently using. I’d ask him about any weight loss or anorexia pulmonary cachexia, which are commonly associated with severe COPD (Kennedy-Malone et al., 2019). It’s important to collect information from the patient regarding his past medical history, family history, history of exacerbations, and impact of symptoms on his activities of daily living and socioeconomic impact. 
What additional objective findings would you be examining the patient for? 
My objective assessment would begin with evaluation of all his vital signs including pulse oximetry and respiratory rate. Physical assessment would include inspection for ruddy or cyanotic skin color, increased anteroposterior diameter of the thorax, use of accessory muscles, and clubbing of nail beds (Cash & Glass, 2020). With auscultation I would assess his cough, lung sounds for prolonged expiration, wheezing or crackles, and decreased heart and breath sounds. Percussion may reveal hyperresonance. Thorough assessment of the cardiovascular system is imperative as it attempts to compensate for the restricted diaphragmatic movement caused by hyperinflation (Kennedy-Malone et al., 2019). I would assess for tachycardia, gallop rhythm, cor pulmonale, jugular venous distension, and peripheral edema.  
What are the differential diagnoses that you are considering? 
The first differential I would rule out is that of bronchiectasis considering the patient reports coughing up blood. Other differentials would include asthma, carcinoma of the lung, interstitial lung disease, and tuberculosis (Hollier, 2018).  
What radiological examinations or additional diagnostic studies would you order? What additional laboratory tests might you consider ordering? 
According to Hollier (2018), the gold standard for diagnosis of COPD is spirometry with bronchodilation. An FEV1/FVC ratio of < 70% constitutes diagnosis in symptomatic patients. Additional testing would include a chest x-ray, sputum testing, CBC (to rule out anemia since our patient complains of dyspnea), ECG, and a PPD. If his pulse oximetry is < 92%, I would check an arterial blood gas. What treatment and specific information about the prescription will you give this patient?  My treatment plan for this COPD patient would begin with continued encouragement for smoking cessation as well as removing exposure to any known respiratory irritants. According to Hollier (2018), most exacerbations of COPD are due to viral infections, including the common cold, so strict hand hygiene and vaccinations (influenza and pneumococcal conjugate) are recommended. Healthy lifestyle habits, exercise training, and proper nutrition are important for nonpharmacological management. Before beginning or modifying the patient’s pharmacological management, I would ascertain information on his current treatment regimen adherence and correct inhaler technique. I would instruct him to discontinue his cough medicine as antitussives are not recommended for the COPD patient. I would use the GOLD guidelines and base his treatment on the algorithm according to his FEV1 and exacerbation history.   Based on the diagnostic studies and sputum testing, if infection is suspected the patient would begin antibiotic treatment. For COPD exacerbation, the patient will be prescribed short-acting bronchodilators (SABDs) and short-term use of oral corticosteroids. Tiotropium, a long acting anticholinergic/antimuscarinic, is used for maintenance to decrease symptoms of dyspnea and reduce exacerbations. What are the potential complications from the treatment ordered?  Beta-2 Agonists-Short Acting (SABAs) selectively stimulate beta-2 adrenergic receptors in the lungs for bronchodilation (Hollier, 2018). The patient should be educated that it may increase heart rate, irritability, blood pressure and QTc interval, as well as lower potassium levels. Complications associated with steroid use include water retention, edema, increased blood pressure, and increased blood glucose levels. Additionally, oral steroids can lower the patient’s immune system. Tiotropium is contraindicated in atropine allergy and requires monitoring of GI/GU obstruction or narrow-angled glaucoma. The patient will be advised to discontinue its use if paradoxical bronchospasms occur. I would have the patient consult a pulmonologist based on the diagnostic study results, severity of symptoms, or if suboptimal response to therapies is noted. Hemoptysis is not uncommon in severe COPD patients and discovering its source is important if it persists. A pulmonologist can order further testing, such as a bronchoscopy to help treat some cases of bleeding.
